In-hospital feedback
If you are currently a patient in hospital with a complaint or a concern, we encourage you to discuss this with your Nurse or Unit Manager.
For all other in-hospital feedback, please complete the short online survey below. Your feedback will remain anonymous unless you choose to include your contact details.
Post-discharge survey
Around two weeks after you are discharged, we will email you a link to our anonymous Patient Experience Survey.
If you would like us to contact you by phone to discuss your experience or feedback, please add your contact details at the end of the survey.
The feedback we receive helps us understand what we are doing well and where we can improve the care we provide to all patients, whānau and visitors.
