Digital portal (preferred method)
Once a date for your surgery has been agreed, you will receive a text invitation from us, asking you to register and complete your hospital admission forms online via our digital portal.
The portal will also provide you with important information and reminders in the lead up to your surgery.
Please contact us if you have not received a text invitation or require assistance to complete these forms through the portal.

If you do not have a cell phone to receive a text invitation to the portal, or do not have a digital device to complete the forms via the portal, you can use the print version of the forms below.
Upon receipt of your forms
After we review your forms, we may contact you to:
- Gather more information about your health
- Request additional documents
- Arrange additional clinical tests
- Arrange a pre-assessment consultation.
